Deliver vital healthcare services directly to patients by conducting home health risk assessments for Medicare Advantage plan members across Leon County, FL. As a Nurse Practitioner in this per diem role, you will complete thorough in-home evaluations in locations including Tallahassee, Woodville, and Midway. Each assessment typically takes about 45 minutes and could range from annual health assessments to post-discharge visits, helping to bridge critical care gaps in the community.

Flexible shifts offered Monday through Sunday. Create your own work schedule. Minimum weekly or bi-weekly hourly commitment required.

Complete comprehensive home health risk assessments for Medicare Advantage members. Review and evaluate patients current and previous medical history, conditions, and medication use. Educate patients on their conditions and prescribed medications. Identify gaps in care, providing recommendations to enhance patient health and safety. No requirements for treatment plan development or medication prescribing.

Active Nurse Practitioner license in Florida. Current CPR certification (BLS required). Board certification through AANP or ANCC. Minimum of six months recent NP experience preferred. MSN degree from an accredited school of nursing.
