You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility's residents.You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.As a Nurse Practitioner, you will provide types of care, including but not limited to:Be comfortable and proficient with documentation and CPT codingEvaluation of new-admission visitsCoordination of discharge visitsMonthly stability visits for long term residentsEvaluations for patients exhibiting changes in conditionAdvanced Care PlanningOther Duties & ResponsibilitiesProvide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and diseasePrescribe all necessary medications and treatmentsPerform comprehensive physical examinations of patientsDevelop and implement patient management policies and proceduresFacilitate referrals to other healthcare professionals and medical facilitiesCommunicate with attending physicians and/or specialists regarding patient careBe comfortable and proficient with documentation and CPT codingRequirementsLicensure / Certification / Registration:Advanced Practice Licensure (APN) with an applicable state licenseMust have an active license and DEA #Previous experience in nursing or other medical fieldsMinimum one-year experience in a Skilled Nursing setting (preferred)Ability to build rapport with patients and their loved onesAbility to thrive in a fast-paced environmentExcellent written and verbal communication skillsStrong leadership experience with emphasis on teamwork in a healthcare settingEducationMaster's degree required
