Work Environment

This position is onsite and primarily involves delivering home care services throughout Fairfax. Working in patients' homes allows our therapists to provide individualized treatments tailored to the unique environments and conditions affecting each patient. You will collaborate closely with an interdisciplinary team, ensuring a comprehensive approach to health care that aligns with each patient's needs.

Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical abilities, functional limitations, and rehabilitation needs. Develop customized treatment plans based on individual patient evaluations.

• Implement evidence-based therapeutic interventions to promote recovery, focusing on restoring mobility, strength, flexibility, and overall functional independence.

• Monitor and document patients' progress through accessible, clear records, ensuring adherence to compliance standards set by state and federal regulations.

• Engage in multidisciplinary collaboration with physicians, caregivers, and other healthcare professionals to optimize patient treatment and outcomes.

• Educate patients and their families on exercises, safety precautions, and adaptive techniques to encourage active participation in their recovery.

• Address and adapt therapy plans in response to changes in patient conditions, using clinical judgment to determine the necessity for additional evaluations or referrals.

• Participate in regular team meetings and training to stay updated on advancements in physical therapy practices and new corporate initiatives.

• Fulfill all required documentation accurately and timely, painting a clear picture of a patient's progress to enhance continuity of care.

Qualifications

Compensation and Benefits
