This role is primarily onsite, as you will be visiting patients in their homes throughout the Centreville area. Key Responsibilities

• Evaluate and assess patients who are recovering from injuries, surgeries, or medical conditions in their homes.
• Develop individualized treatment plans, employing a range of therapeutic techniques such as exercises, hands-on therapy, and the use of specialized equipment to assist in pain relief and mobility improvement.
• Implement treatment strategies and monitor patient progress, documenting all findings and outcomes to ensure the efficacy of care.
• Regularly reevaluate objectives and adjust treatment plans as necessary to align with patient goals and enhance recovery outcomes.
• Educate patients and their families on exercises and mobility techniques, making certain that they are equipped with the knowledge they need for effective self-care.
• Work collaboratively with other healthcare professionals to ensure a holistic approach to patient care, including providing training and support for caregivers.
• Stay updated on the latest advancements in Physical Therapy through workshops and professional development opportunities.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice Physical Therapy.

Preferred Experience
