Work Environment

As part of our home care team, you will have the unique opportunity to work directly in patients' homes, which allows for personalized assessments and treatments tailored to the nuances of their daily lives. The full-time position offers a structured schedule from Monday to Friday, 8:00 AM to 5:00 PM, allowing for a balanced work-life dynamic. Key Responsibilities

• Conduct comprehensive evaluations and assessments to determine the physical therapy needs of each patient.
• Design and implement individualized treatment plans that address functional limitations and facilitate the restoration of mobility.
• Monitor patient progress, documenting treatment outcomes and modifying plans as necessary in accordance with each patient's evolving needs.
• Consult effectively with physicians and other healthcare providers to ensure a cohesive approach to patient care.
• Document clinical findings accurately and in a timely manner, including detailed initial evaluations, progress notes, and discharge summaries, to maintain compliance with state regulations and corporate standards.
• Engage actively in discharge planning and provide continuity of care by ensuring appropriate follow-up is arranged.
• Uphold our service quality standards while striving to maximize productivity and efficiency in all patient interactions.
• Participate in ongoing training to maintain knowledge of the latest industry practices and healthcare advancements.
• Attend interdisciplinary meetings and contribute to the team's efforts in improving patient access and satisfaction.

Patient Population / Client Focus

You will primarily work with patients dealing with a range of orthopedic conditions, including post-surgical rehabilitation, chronic pain management, and musculoskeletal disorders. Our emphasis on personalized care means you will establish strong rapport with clients, fostering trust and collaboration in their recovery journey. You will also modify treatment techniques according to individual patient responses, ensuring a patient-centric approach at all times.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Active and valid state physical therapy license.
• A minimum of two years of clinical experience in a relevant physical therapy setting, particularly in orthopedic rehabilitation.
• Current CPR/BLS certification.
• Strong clinical assessment and treatment planning skills, with the ability to effectively engage with patients and their families.

Preferred Experience

• Experience in home care settings is highly desirable, allowing for the adaptation of treatment in real-world environments.
• Training in therapeutic modalities and evidence-based practices relevant to orthopedic therapy.
• Familiarity with digital health tools for patient management and engagement would be an asset.

Compensation and Benefits

This position offers a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience and qualifications.
