Work Environment

This position is onsite and primarily focuses on home care, allowing you to work directly within patients' living environments. This approach not only helps in tailoring treatments based on real-life scenarios but also fosters stronger patient-therapist relationships. Our environment promotes flexibility, with Monday through Friday schedules that average 40 hours a week, allowing for a healthy balance between professional responsibilities and personal life.

Key Responsibilities

• Deliver safe, effective, and compassionate patient care tailored to individual needs across various orthopedic conditions, ensuring adherence to best practices in physical therapy.

• Implement comprehensive treatment programs designed by your supervisory physical therapist, focusing on restoring functionality and mobility for your patients.

• Conduct thorough evaluations, assessing patient history and conducting necessary assessments to establish a solid treatment plan and document findings accurately.

• Manage discharge planning and follow-up care, ensuring clear, concise documentation of each patient's care journey, including baseline assessments, progress notes, and discharge evaluations.

• Maintain compliance with all corporate training requirements and regulatory standards, and participate in continuing education opportunities to stay current with industry advancements.

• Establish and maintain open lines of communication with colleagues, physicians, patients, families, and third-party payers to promote cooperation and holistic patient care.

• Actively engage in facility meetings and contribute to our team discussions to promote ongoing professional development and share best practices.

Patient Population / Client Focus

Your primary focus will be on orthopedic populations, handling cases that can range from post-surgical rehabilitation to chronic injury management. The home care setting allows for direct observation of patients' daily living challenges, enabling you to create tailored therapeutic interventions aimed at improving their overall functional status and quality of life.

Qualifications

• Graduate from an accredited Physical Therapy program.

• Doctorate in Physical Therapy (DPT) is required.

• A valid state license to practice physical therapy and specialty certifications as deemed necessary.

• Current CPR certification through the American Heart Association.

Preferred Experience

• A minimum of 2 years of experience in an orthopedic setting, with a demonstrated track record of successful patient outcomes.

• Experience in home health physical therapy is advantageous but not mandatory.

Compensation and Benefits
