Work Environment

This full-time position will require the selected individual to work onsite, primarily in the homes of our patients. You will have the opportunity to make significant contributions in a flexible schedule, primarily Monday to Friday, amounting to 40 hours a week. Key Responsibilities

• Deliver high-quality, compassionate physical therapy care tailored to the individual needs of patients in their home environments, ensuring safety and comfort.

• Implement treatment programs developed collaboratively with fellow practitioners, adhering to established plans of care that drive individual patient progress.

• Conduct comprehensive evaluations that include thorough history assessments, targeted diagnostic testing, and clear documentation of results to guide treatment decisions.

• Manage discharge planning and follow-up procedures effectively, ensuring that documentation accurately reflects the patient's journey, including progress notes and evaluation summaries.

• Stay compliant with state and federal regulations while maintaining corporate training requirements relevant to clinical services.

• Establish effective communication with colleagues, physicians, patients, and families, ensuring transparency and understanding throughout the treatment process.

• Engage in regular team meetings and uphold an environment of collaboration and respect among health professionals.

Patient Population / Client Focus

The primary focus of this role is providing care to diverse orthopedic populations, including patients recovering from surgery, managing chronic pain conditions, and undergoing rehabilitation from injury. Your ability to implement tailored treatment plans for individuals in their home settings will directly impact their recovery and overall health outcomes, making a meaningful difference in their lives.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.

• Active licensure to practice physical therapy in the State of Virginia.

• Valid BLS for Healthcare Providers CPR certification from the American Heart Association.

Preferred Experience

Successful candidates will have a minimum of two years of experience as a licensed physical therapist. Experience in home health or outpatient orthopedic settings is highly desirable. Familiarity with electronic health record (EHR) systems as well as multidisciplinary collaboration to enhance patient care outcomes would be beneficial.

Compensation and Benefits
