Overview Position: Clinical Case Manager Reports to: Program Supervisor The Clinical Case Manager provides trauma-informed, clinically-oriented case management services to adults with serious mental health conditions who are transitioning from incarceration, probation, or other justice system involvement back into the community. This role integrates clinical assessment, care coordination, and reentry stabilization services, supporting clients in navigating reintegration challenges including housing instability, behavioral health needs, substance use recovery, and access to medical and social services. The Clinical Case Manager applies clinical judgment, risk assessment, and crisis intervention skills to promote safety, reduce recidivism, and support long-term recovery. Services are delivered in the field (homes, correctional reentry settings, and community agencies) as well as via telehealth. Working as part of a multidisciplinary team, the Clinical Case Manager collaborates with probation/parole, behavioral health providers, housing partners, and community-based organizations to ensure coordinated, client-centered care. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture. The schedule for this role is Wednesday-Sunday 8:00 a.m. - 8:00 p.m. Responsibilities Conduct comprehensive clinical and reentry-focused assessments, including mental health, substance use, criminogenic risk factors, housing stability, and psychosocial functioning. Perform and document mental status examinations (MSEs) and ongoing risk assessments, including evaluation of safety concerns, relapse risk, and recidivism factors. Develop and implement individualized, recovery-oriented service plans that address both clinical needs and successful community reintegration goals. Provide intensive, field-based case management to support clients during critical transition periods following release from incarceration or institutional settings. Deliver crisis intervention, de-escalation, and safety planning, coordinating higher levels of care (e.g., hospitalization, detox programs) when clinically indicated. Support clients in accessing behavioral health treatment, including linkage to psychiatric services, therapy, and substance use recovery programs. Collaborate with probation/parole officers, reentry programs, correctional facilities, and legal system partners to ensure compliance with conditions of release while supporting client rehabilitation. Assist clients with securing stable housing, including transitional housing, reentry housing programs, shelters, and independent living placements. Facilitate access to benefits and entitlements (e.g., SSI/SSDI, Medi-Cal, CalFresh), identification documents, and employment or vocational services. Provide rehabilitation counseling and skill-building interventions, including life skills, coping strategies, and community integration support. Coordinate care with medical and dental providers, ensuring continuity of care for co-occurring physical health conditions. Maintain accurate, timely, and compliant clinical documentation, including progress notes, treatment plans, ANSAs, and reentry-related outcomes. Participate in multidisciplinary team meetings, case conferences, and clinical supervision, contributing clinical insight on high-risk and complex cases. Build and maintain strong relationships with community reentry resources and service providers to expand support networks for clients. Provide transportation support and accompaniment to appointments when necessary to ensure engagement in services. Additional Clinical & Reentry Competencies Serve as a liaison with funders regarding programmatic and operational issues. Attend all funder-required meetings, events, and trainings. Ensure delivery of program elements in compliance with contract language. Submit all required documents and reports by the deadlines. Collaborate with La Familia Administration and the Supervisor to ensure accurate and appropriate contract expenditures. Knowledge of criminogenic risk factors, recidivism reduction strategies, and justice system processes. Experience applying trauma-informed care and culturally responsive practices, particularly with historically marginalized and justice-involved populations. Ability to balance accountability (legal compliance) with client-centered, recovery-oriented care. Strong skills in motivational interviewing, harm reduction, and relapse prevention. Other Attend trainings and participate in events relevant to key responsibilities. Track and regularly report on key metrics for responsible function areas. Lead and collaborate on special projects related to agency objectives as appropriate. Qualifications & Experience BA degree with minimum two years of experience in behavioral health, reentry services, or working with justice-involved populations, or MA in a related clinical field. Experience with individuals with serious mental illness, substance use disorders, and/or co-occurring conditions. Pior experience in reentry, forensic mental health, corrections, or probation/parole collaboration strongly preferred. Bilingual (English/Spanish) highly desirable. Strong clinical documentation and assessment skills. Experience in non-profit and community-based organizations preferred. Demonstrates excellent customer service and ability to work with diverse populations; discretion with confidential information. Proficient in MS Office and Google Workspace. Valid CA Driver’s License and willingness to provide field-based services. Other Responsibilities Demonstrates cultural awareness and provides respectful, client-centered care. Maintains knowledge of community resources for client referrals. Attend work regularly and adhere to policies on absences and tardiness. Understands and applies legal requirements, including confidentiality and risk management. Timekeeping & Attendance Accurately record work hours each day by clocking in/out at scheduled start, break, lunch, and end times. Take required meal and rest breaks in compliance with state, federal, and agency policies. Review and submit timesheets by deadlines to ensure timely payroll processing. Notify supervisor of scheduling conflicts, absences, tardiness, or timekeeping issues. Maintain consistent and reliable attendance to support team and program operations. Follow policies related to overtime, schedule changes, and paid/unpaid leave. Report timekeeping discrepancies to the supervisor for resolution.
