Your future role at a glance Location: Chicago, IL Facility: Ascension Saint Joseph Department: Adult Inpatient Unit Schedule: Day shift | Full-time Salary: $63,363.30 - $92,668.83 per year How you'll make an impact in this role Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Create plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations and/or patient records. Advocate for resources and removal of barriers. Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. Act as a resource for adequate medical record documentation, appropriateness of services as they relateto diagnoses, and treatment options for post-discharge care. What minimum qualifications you'll need Licensure / Certification / Registration: One or more of the following required: Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Education: High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
