Key Responsibilities Conduct comprehensive assessments to identify the specific mental health needs of patients, utilizing evidence-based approaches for effective evaluation. Maintain clear and ongoing communication with supervisors, direct care staff, and relevant stakeholders, providing updates on patients’ progress. Develop and deliver a structured schedule for appointments, in-home visits, and observations to monitor treatment progress and program implementation. Organize and coordinate home visits, conducting necessary chart reviews and interviews, as well as gathering data for ongoing analysis of treatment efficacy. Ensure that appropriate treatment plans are executed effectively, regularly adjusting strategies based on patient response and feedback. Participate actively in interdisciplinary team meetings to collaborate on patient care, share insights, and contribute to program development. Document monthly and quarterly progress notes to track patient improvements and challenges systematically. Design and facilitate training programs to ensure that staff members are fully competent in understanding and implementing treatment protocols and documentation practices. Provide ongoing follow-up support through interviews and home visits to address evolving patient needs. Act as a liaison within the healthcare community to enhance outreach efforts and promote our development goals. Assist in creating and refining training materials to continuously improve our therapeutic programs for diverse populations. Patient Population / Client Focus In this role, you will work with a varied population, encompassing both adults and youth who present with a broad spectrum of mental health issues. From those experiencing trauma and addiction to families needing support for relational dynamics, you will play a critical role in guiding them towards recovery and healthier living. Your specialized knowledge in family therapy, couples therapy, or trauma-informed care is highly valued and encouraged in this position. Qualifications A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory. An active and unrestricted LPC licensure in Arizona is required. A minimum of two years of experience in counseling, preferably in a clinical context, is essential. Specialization in areas such as family therapy, couples therapy, or trauma is strongly preferred. A solid understanding of the legal and ethical standards surrounding mental health practice. Exceptional interpersonal and communication skills to build rapport with diverse clients. Proven ability to work collaboratively with a multidisciplinary healthcare team. Familiarity with EMR software and other healthcare technology. Preferred Experience Experience developing treatment plans and documenting patient progress in a clinical setting. History of conducting family and couples therapy sessions effectively. Background in substance misuse counseling, trauma-informed care, or similar specialties is a strong asset.
