Actify Home Care is clinician-owned and operated by a physical therapist with over 20 years of experience. We understand the physical and clinical demands of field-based SLP therapy because we’ve been in your shoes. We aren't just an agency; we are a clinical team that values your expertise.
Actify Home Care is growing, and looking for Geriatric SLP home health therapists to join our team in West Central Broward County: Tamarac, Sunrise, Plantation, Fort Lauderdale, and surrounding areas.
SLP Per-Visit Compensation (W-2 Employee):
Evaluation / Re-Admission: $100.00
Routine Visit: $80.00

What You’ll Do:
Provide high-quality, one-on-one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive-linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health.
Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders.
What You Need:
License: Active Florida Speech-Language Pathologist (SLP) License.
Certification: Current CPR/BLS Certification.
Transportation: Valid Driver’s License and reliable transportation.
Software: Kinnser/WellSky experience is a major plus!
