Position Summary The Public Health Nurse (PHN) provides nursing services to individuals, families and population groups. Nursing activities include but is not limited to, reproductive and sexual health services, utilization of current ACIP immunization recommendations, clinical services, care coordination services, disease investigation and control measures, child fatality prevention services, analysis of health data, and coalition building and collaboration with community partners. The PHN is generally assigned to one to two programs but are also expected to provide PHN services and functions as assigned. Essential Job Functions Clinical Services Performs assessments and provides interventions to individuals seeking reproductive and sexual health services including focus reproductive health exams, contraceptive counseling (including emergency contraception), STI testing and treatment, and health education. Administer immunizations to infants, children, and adults following ACIP guidelines. Conduct international travel consultations and provide related immunizations and preventive medications. Deliver nursing care and interventions in collaboration with pediatric specialty providers. Perform directly observed therapy (DOT), field-delivered therapy, and communicable disease prevention activities as needed. Care Coordination & Case Management Assess client needs and coordinate care with other health professionals. Authorize services, make referrals, and ensure appropriate follow-up. Provide nursing consultations and recommendations to support medical needs. May involve home visits or travel within Colorado. Community Health & Outreach Provide outreach and nursing support to promote community health and wellbeing. Obtain and effectively use community resources. Analyze child fatality data and collaborate with partners to develop prevention strategies. Assist in training new employees and support community education efforts. Safety, Compliance & Documentation Maintain clean and safe exam rooms and work environments in compliance with legal and procedural standards. Complete accurate and confidential charting using paper and electronic systems. Exercise independent judgment, within RN scope of practice, in attaining program goals and activities while adhering to established protocols and seek supervision when needed. Professional Development & Agency Support Participate in continuing education to ensure evidence-based care. Contribute to strategic planning, performance management, quality improvement, and policy development. Engage in emergency response training and drills using the incident command structure. Perform other duties as needed to support program and agency goals. Responds to public health emergencies, as needed, and directed, aside from typical duties and schedule for the position. Knowledge | Skills | Abilities Knowledge Best practices and policies relevant to registered nursing. Prescribed treatments and medical interventions, including monitoring and documentation of patient progress and referrals. Chart documentation requirements, medical record forms, and laws governing healthcare information and client consent. Systematic approach to client care: assessment, diagnosis, planning, intervention, evaluation, consultation, referral, and follow-up. Office procedures including telephone etiquette and use of office equipment (e.g., copier, fax machine, computer, scales). Skills Excellent verbal and written communication. Records management and documentation accuracy. Computer proficiency, including use of software, Microsoft excel, keyboarding, and troubleshooting. Organization and efficiency in managing multiple responsibilities. Attention to detail in chart updates and program documentation. Qualifications | Requirements Required Education: Associate Degree in Nursing or Nursing Diploma required. Bachelor of Science in Nursing preferred. Must possess current licensure as a Colorado Registered Nurse. Experience: One year of nursing experience as a registered nurse preferred. Public health experience is also preferred. Fluency in writing, texting, and speaking English. Ability to write, text, and speak fluently in Spanish is desired. Must possess and maintain a valid Colorado driver’s license and be able to provide personal transportation for work-related business. Must obtain annual flu vaccination and obtain any job specific immunizations. Current CPR certification (Healthcare Provider level) required at time of hire. Required to submit to a TB test. Work Environment | Physical Demands Work schedule is typically Monday through Friday, 8:00 a.m. to 5:00 p.m. but may be required to work beyond normal work schedule. The work may require some physical exertion such as long periods of sitting, standing, recurring bending, crouching, stooping, stretching, reaching or similar activities. May be required to lift and carry up to 49 pounds on an infrequent basis lift and carry up to 20 pounds on a regular basis, climb stair, bend, reach, hold (particularly children receiving vaccines – may require ample pressure), grasp and turn objects; and use fingers to perform routine medical procedures and administer vaccines, operate computer or typewriter keyboards. The work requires the ability to speak fluently and to use normal or aided vision and hearing. Work is subject to recurring and inflexible deadlines and frequent interruptions; may involve evening and weekend meetings/trainings and travel out of the area.
