Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, your primary responsibilities will include:

• Directing physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and restoring function to clients in accordance with their individualized care plans.
• Evaluating clients' physical abilities and levels of function through thorough assessments, diagnostic tests, and functional ability tests.
• Overseeing the implementation of care by physical therapy assistants, ensuring adherence to established treatment protocols and quality standards.
• Administering manual therapy techniques, therapeutic exercises, and utilizing advanced modalities for effective pain management and functional improvement.
• Regularly monitoring and documenting patient progress, adjusting treatment plans as necessary and consulting with physicians and interdisciplinary teams to optimize patient outcomes.
• Providing education to clients and their families regarding rehabilitation exercises, device usage, and self-management strategies to encourage independence.
• Actively participating in team meetings, case discussions, and contributing to the evolution of care plans based on patient feedback and clinical observations.
• Maintaining accurate clinical records, including evaluations, care plans, and progress notes, to ensure continuity of care and comply with agency policies.

Patient Population / Client Focus

In this role, you will primarily serve clients requiring outpatient rehabilitation services. The patient demographic includes individuals recovering from surgeries, sports injuries, chronic pain conditions, neurological disorders, and post-traumatic rehabilitation. Qualifications

A successful candidate will possess the following qualifications:

• Bachelor's degree in Physical Therapy or Doctorate degree (DPT) from an accredited institution.
• Current and valid physical therapy license in Florida.
• At least two years of clinical experience in physical therapy, preferably in an outpatient setting.
• Current CPR certification.
• Proficient understanding of federal, state, and local healthcare regulations governing physical therapy practice.

Compensation and Benefits
