About the Role:
As a Clinical Liaison at Valley Manor, you will play a vital role in coordinating patient care, streamlining discharges, and fostering strong relationships with healthcare providers. Your primary responsibilities will include:
• Coordinating patient care: Collaborating with healthcare providers, case managers, and families to ensure comprehensive care plans are in place, and patients receive the necessary support for their recovery.
• Streamlining discharges: Facilitating efficient discharges by coordinating with providers, scheduling transportation, and ensuring patients have the necessary resources for a smooth transition home.
• Fostering strong relationships: Building trust with healthcare providers, patients, and families by being empathetic, approachable, and knowledgeable about patient needs.
• Communicating effectively: Maintaining open lines of communication with internal and external stakeholders to ensure seamless coordination of patient care.
