This position will be remote, allowing for flexibility while remaining integral to our hospice center operations. Working with a variety of healthcare professionals, including medical doctors, registered nurses, and support staff, you will play a vital role in developing patient-centered care plans that directly impact the quality of life for our patients and their families. • Conduct thorough mental health assessments that examine the emotional, psychological, and social needs of hospice patients, ensuring all factors are thoroughly understood.

• Develop and implement individualized treatment plans in collaboration with the healthcare team and based on patient assessments, focusing on addressing mental health challenges and enhancing emotional wellness.

• Provide evidence-based therapeutic interventions—including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and other modalities—tailored to the unique needs of patients coping with end-of-life issues.

• Facilitate individual and group therapy sessions that work to strengthen emotional resilience, help patients articulate their feelings, and develop effective coping strategies.
\n
• Collaborate closely with other health professionals to ensure a holistic treatment approach, sharing insights and updates to optimize patient care.
\n
• Maintain accurate and timely records of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.
\n
• Participate in regular team meetings to discuss case reviews, share strategies, and enhance service delivery to improve patient outcomes.

In this role, you will primarily serve patients in hospice care, who may be struggling with anxiety, depression, grief, and other complex emotional issues related to their health conditions. This position requires sensitivity and a deep understanding of the unique challenges faced by patients nearing the end of life, as well as by their families. You will help patients navigate their emotional journeys during this critical phase, facilitating both personal and family healing.

• A master’s degree in social work from an accredited institution and current LCSW licensure is required.

• Comprehensive understanding of mental health theory, practices, and interventions specific to hospice care settings.

• Excellent communication and interpersonal skills, with the ability to establish rapport with patients and colleagues.

• Strong diagnostic and assessment skills to identify mental health needs effectively.

• Experience in developing treatment plans and providing therapy within an interdisciplinary team structure.

• A minimum of 2 years of clinical experience in a mental health setting, with specific experience in hospice care or palliative settings preferred.

• Familiarity with various therapeutic modalities and a strong desire to stay current with best practices in mental health care.

• Proficiency in electronic health records (EHR) and teletherapy practices will be considered an asset.
