Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and develop customized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional capacity
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate and guide patients and their families on therapeutic goals, discharge planning, and exercises for home practice
• Maintain precise documentation of evaluations, treatments, and progress per facility and regulatory guidelines
• Participate in care team rounds and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are welcomed but not mandatory

Skills

• Prior experience is highly preferred
