Position Summary:

The Behavioral Health Case Manager (BHCM) focuses on providing behavioral health intervention, and other recovery support services to adults with substance use disorder (SUD) and/or co-occurring disorders in the communities deemed high-risk opioid overdose as a key component of the Mobile Outreach and Recovery Services (MORS) Continuity of Care (COC) team.

The BHCM plays a vital role in ensuring that individuals in need of behavioral health care and recovery support services receive comprehensive, coordinated, and person-centered care. By collaborating with Certified Peer Specialists/Certified Recovery Specialists and other members of the COC team, BHCM’s strive to improve outcomes and promote the overall well-being of individuals seeking behavioral health services in the community. The COC team works to ensure warm handoffs, smooth transitions, and continuity of care for individuals seeking support.

As a part of the warm hand-off process, the BHCM conduct assessments, facilitate, and ensure linkages to care/services, and maintains accurate records to help individuals access services across a continuum of care to achieve their health and wellness goals in the community. This position requires telephonic outreach, progress notes, data entry, and periodic reports.

This position is based at designated locations in communities deemed high risk for substance misuse and heroin/opioid overdose such as Kensington and other “hot spots” in Philadelphia County. No less than 75% of the time shall be spent conducting outreach, engagement, and case management to assist individuals seeking care and supportive services as needed. The BHCM must also be able to work closely with other multidisciplinary teams and organizations in the community.

Duties and Responsibilities:

• Provide assertive outreach and engagement to the adults who are in the epicenter of the opioid epidemic within Philadelphia County.
• Coordinate and provide case management that is safe, timely, effective, efficient, equitable, and client-centered on a regular basis.
• Review client-level information to determine eligibility, risks, needs, and level of care in accordance with ASAM placement criteria.
• Manage case assignments, draft service plans, review case progress, and determine case disposition and discharge plans as appropriate
• Record and maintain case information, accurately complete all necessary forms, and draft reports as needed.
• Coordinate, monitor, and track warm handoffs as individuals are screened, placed/referred to clinical treatment and/or recovery support services throughout the continuum of care.
• Develop and maintain effective working relations with multidisciplinary teams and stakeholders throughout the case management process.
• Adhere to professional standards as outlined by protocols, internal policy and procedures, relevant rules and regulations, and case management best practices.

Skills Required:

Knowledge of:

• American Society of Addiction Medicine (ASAM) criteria
• Behavioral health and addictions services (inpatient detoxification, medicated assisted treatment (MAT) ambulatory detoxification, etc.)
• Case Management Principles and Standards

Education and Experience:

• Master’s degree in social work, human services, or related field with minimum 2 years’ experience in addictions; or
• Bachelor’s degree in social work, human services, or related field with minimum 5 years’ experience in addictions field.
• Valid Drivers’ License
