Location: GeoPod: Ventura County: Oxnard, Camarillo, Thousand Oaks, Moorpark
Schedule: Per-Diem

As we grow, we're looking for compassionate, licensed Occupational Therapists (OTs) who are ready to deliver patient-centered care directly to clients in their homes.

As a Home Health Occupational Therapist , you'll help patients:
Regain independence through customized therapy plans
Improve functional abilities for activities of daily living (ADLs)
Recover from injury, surgery, or hospitalization in the comfort of home
Reduce fall risk and improve home safety
Your Day-to-Day Includes:
Performing in-home OT evaluations and reassessments
Creating individualized care plans tailored to each patient's goals
Collaborating with nurses, physical therapists, and physicians
Educating patients and caregivers on proper techniques and safety
Documenting patient progress in compliance with state and Medicare guidelines
Licensed Occupational Therapist in the state of California
Graduate of an ACOTE-accredited OT program
Minimum 1 year of clinical OT experience (Home Health experience preferred)
Familiar with electronic documentation systems (EMRs)
Excellent communication and time management skills
Valid driver's license, auto insurance, and reliable transportation
