Essential Functions and Responsibilities:
- Psychotherapy with people in skilled nursing, assisted living, and long term care settings.
- Deliver high-quality, patient-centered care rooted in best practices.
- Collaborate with an interdisciplinary team of medical and psychiatric providers to optimize quality-of-life and support facilities.
- Support patients with conditions including depression, anxiety, adjustment disorders, dementia, grief, loss, and life transitions.
- Timely, accurate documentation.
Financial Rewards and Benefits:
- Annual compensation $55,000.00.

Qualifications:
- Unrestricted LCSW or LPC-MHSP license.

- Experience working with geriatric population and persons with dementia preferred.
- Experience working in a post-acute or healthcare setting is preferred.
