Req #: 0000255645 Category: Case Management Status: Full-Time Shift: Day Facility: Jersey City Medical Center Department: RW Part A - EHA Pay Range: $28.63 - $32.93 per hour Location: 355 Grand Street, Jersey City, NJ 07302 Job Title: Case Manager Job Overview Responsible for providing comprehensive medical case management, health, and psychosocial education to CCC patients, families and significant others. Qualifications Bachelor’s Degree required in Social Work or a related field. Demonstrates knowledge of case management standards. Demonstrates the ability to function as a member of an interdisciplinary team. Preferred Master’s Degree in a mental health discipline that terminates in licensure strongly preferred. Experience and knowledge of HIV, entitlement programs, and community resources. Essential Functions Conduct risk reduction, adherence counseling, and provide referral for services to patients with the goal of preventing HIV transmission. Provide counseling and information regarding advance directives, and assist in the completion of forms. Refer and link patients to appropriate services that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Maintain up-to-date records of all patient encounters and necessary documentation in the chart. Monitor and complete timely documentation to ensure follow-up of all referrals. Provide access to emergency financial assistance (e.g., food vouchers, utility payment assistance, and transportation) as needed to promote and maintain positive health outcomes. Provide supportive counseling as needed and refer appropriate patients to the mental health counselor. Maintain HIPAA guidelines and confidentiality when interacting with patients, families, personnel, and the general public. Respond in a timely manner to verbal and written requests from patients, interdisciplinary team members, and administration, medical and public agencies. Attend scheduled medical case management trainings. Attend scheduled medical case management meetings. Comply with unit and departmental scheduling guidelines. Promote and foster a positive work environment. Perform other duties as assigned.
