Responsibilities include: Coordinate and facilitate the admissions process for both campuses. Work closely with families, residents, patients, hospitals, physicians, healthcare providers, and referral partners to ensure positive transitions into Nazareth Home. Develop and maintain a strong referral network and cultivate professional relationships with healthcare and community partners. Successfully manage communication and relationships with prospective residents, patients, families, and referral sources. Ensure timely, accurate, and professional communication throughout the admissions process. Plan and organize the program of work for the department. Determine work procedures, prepare schedules, and expedite workflow to ensure department objectives are met. Assign duties and monitor work for accuracy, efficiency, and compliance with established policies and procedures. Identify opportunities to improve the admissions process and enhance the experience for residents and their families. Collaborate with leadership and other departments to support organizational goals and census growth. Maintain accurate records and documentation related to admissions and referrals. Experience in healthcare, long-term care, senior living, admissions, marketing, sales, or a related field preferred. Strong communication and interpersonal skills. Strong leadership, delegation, and organizational skills. Excellent relationship-building and customer service abilities. Strong computer skills and proficiency with common office applications. Ability to multitask, prioritize, and work effectively in a fast-paced environment. Strong attention to detail and commitment to accuracy. Ability to make sound decisions and solve problems independently. Commitment to providing an exceptional experience for residents, patients, and their families. Demonstrated ability to work collaboratively with healthcare professionals, families, referral partners, and internal teams.
