Help residents adjust to the skilled nursing environment and address social and emotional needs. Assist with completing and maintaining resident documentation and records. Communicate with residents, families, staff, and outside resources as needed. Assist with care plan meetings and follow-up activities. Help coordinate appointments, referrals, and community resources. Assist with discharge planning and transition-related activities. Support resident rights, dignity, privacy, and quality of life. Participate in interdisciplinary team meetings and facility activities. Maintain confidentiality and follow all facility policies and applicable regulations. Qualifications Previous experience in a healthcare, long-term care, social services, or customer-service environment preferred. Strong communication and interpersonal skills. Compassionate and patient approach when working with residents and families. Excellent organization and attention to detail. Ability to work effectively as part of an interdisciplinary team. Proficiency with basic computer applications and documentation. High school diploma or equivalent required; additional education in social services, human services, psychology, or a related field preferred. Make Everyday Matter.
