Field RN Care Manager In-Home Health & Care Coordination

Humana Inc is looking for RN, Field Care Managers in Chesapeake, Virginia to perform telephonic and face-to-face assessments of our LTSS and ABD member population. In this role, you will engage with members, connecting them with necessary resources for their health and well-being, and developing individualized care plans. This position requires a valid RN license and offers competitive benefits including health insurance and retirement plans.
