Job Description Job Responsibilities: Conduct Initial and continuing assessment of each participant and development of a written plan or care. Provide restorative and supportive nursing as indicated by the participants’ treatment plan. Monitor, administer, and record prescribed medications. Monitor and record vital signs. Supervise personal care given by Program aide. Maintain participant’s health record which includes: quarterly progress notes, records of medications and treatments administered, progress notes, flow sheets. Liaison with PCP, caregivers and family members.
