Hospice Visit Nurse (License Vocational Nurse or Registered Nurse)

Job Overview Anchor Health is looking for a registered nurse/ licensed vocational nurse to provide hospice care services. This role involves planning, coordinating, and delivering patient‑centered care under the direction of the Director of Patient Care Services. Responsibilities Provide professional nursing care to patients using nursing process elements. Assess and evaluate patient status. Initiate and modify care plans based on patient status and needs. Develop care plans incorporating nursing diagnoses and palliative actions, involving patients and families. Work Tuesday‑Saturday, 12 PM‑8 PM; position is PRN. Provide care across multiple service areas (Alameda, Contra Costa, Napa, Santa Rosa, Solano, Sonoma). Act as case manager when assigned, coordinating patient care for assigned caseload. Complete, maintain and submit accurate clinical notes, pain/symptom management records. Communicate with physicians regarding patient needs, report changes, obtain orders. Coordinate care with community health professionals. Qualifications Graduate of a National League of Nursing accredited school of nursing. Current licensure in State of CA and CPR certification. Minimum of one year of RN/LVN experience within last three years; OR baccalaureate degree in nursing from an accredited program and current RN/LVN license. Preferred: minimum of two years experience, at least one in public health, home care, or hospice nursing. Management experience not required; responsible for supervising hospice aides. Excellent observation, verbal and written communication skills, problem‑solving, basic math skills, nursing skills per competency checklist. Physical ability to walk, stand, lift, transfer patients and equipment; handle reaching, stooping, bending, kneeling, crouching. Licensed driver with a safe, insured automobile in good working order.
