Job Summary:

As a Social Worker, you will play a vital role in providing psycho-social support to our residents and their families. You will assess the emotional, social, and psychological needs of our residents and develop individualized plans to address their concerns. Your expertise will help ensure that our residents receive compassionate and respectful care that honors their dignity and autonomy.

Responsibilities:

• Assess the emotional, social, and psychological needs of residents and develop individualized plans to address their concerns
• Provide psycho-social support to residents and their families, focusing on emotional, social, and psychological well-being
• Collaborate with other healthcare professionals to develop and implement personalized care plans
• Identify and address any emotional, social, or psychological issues that may impact a resident's recovery or quality of life
• Maintain accurate and detailed records of resident interactions and care plans
• Participate in interdisciplinary team meetings to share knowledge and expertise, and to ensure seamless communication among team members

Requirements:

• Bachelor's degree in Social Work
• Minimum 1 year of experience in social work or a related field
• Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and healthcare professionals
• Ability to work effectively in a fast-paced, dynamic environment
• Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
• Compassionate and empathetic demeanor, with a commitment to delivering exceptional care to our residents
