Case Manager

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability.


Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.


Job Summary

Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is seeking a Housing Focused Case Manager. The Housing Focused Case Manager assists individuals exiting homelessness and entering housing with obtaining and retaining permanent housing and financial stability. All activities with clients are consistent with established housing and other goals. The case manager engages clients by establishing trust and collaborative relationships directed toward achieving goals assist with identifying and overcoming social, supportive, educational, and personal barriers to accessing and maintaining housing and provide direct referrals for community resources and avenues for partnerships with natural supports. The core working hours of St. Patrick Center is Monday through Friday from 8am to 4:30pm. Starting compensation is $45,000.

Overview and Responsibilities

• Correspond with lawyers and judges and advocate for client inside a courtroom setting and attend monthly meetings and other pretrial events, as needed.
• Make in home visits to observe indicators of clients exiting program. Visting clients in a jail setting, if needed.
• Assist clients with creating goals in short & long-term housing and financial stability.
• Utilize client strengths and preferences as foundation on which to build goals.
• Develop housing stability plan with client, other treatment providers, family members and community supports with bi-monthly updates.
• Provide education and support to family members about work with permission of client.
• Gather input about skills, interests, strengths, etc. of client and ideas for establishing a system of support.
• Assist clients with financial stability including but not limited to obtaining employment and/or government benefits.
• Provide outreach services to client when appointments are missed or appear to disengage in services by using variety of methods to discover interference with housing stability.
• Create budget plan with all clients and update when housed as necessary due to changes in income and/or expenses.
• Review and/or prepares check requests and gathers supporting documentation for client support services.
• Refer client to service line programs and community service providers as needed.
• Utilize and coordinate housing efforts with case managers and other staff service line and in community.
• Maintain detailed records of client services/expenditures and closely monitor expenditures to prevent over or under spending of funds.
• Maintain and update daily the required documentation paper and electronic files of clients.
• Accurately enter client activity and provide services in Case Worthy within 1 business day of activity/service.
• Compliance with HIPAA regulations.
• Foster strong working relationships with community-based organizations and partner agencies.
• Provide consistent communication with partner agencies/organizations to ensure coordination of services.
• Maintain monthly outcomes and data tracking.
• Participate in program team meetings.
• Communicate individually with coworkers between meetings to coordinate services and generate ideas to help achieve housing goals of clients.
• Assist manager in training new program coworkers.
• Outcomes: 85% of those served exit program to stable permanent housing with positive experience.
• Outcomes: 60% of those served increase their total income.
• Other duties as assigned.

Knowledge and Experience Requirements

• Requires bachelor's degree.
• Degree in Human Services, Counseling, Social Work, or related field preferred.
• Prior experience working with socially/economically disadvantaged persons and assisting with housing placement and landlord development activities preferred.
• Requires driver's license (Missouri residents require Class E), personal vehicle, and auto insurance.
