Behavioral Health Case Manager — Bilingual Care & Access Advocate

ZUFALL HEALTH CENTER INC

Zufall Health Center Inc. is seeking a Case Manager who collaborates with the care team to help patients access clinical and social services. You will address barriers such as finances, language, and insurance, and help coordinate referrals and transportation to appointments. The role requires experience in a clinical setting, bilingual English/Spanish, and comfort navigating EMR systems.
