Hospital Liaison

Market Liaison Skilled Nursing & Post-Acute Care

Champion Care is seeking a driven and relationship-focused Market Liaison to support our skilled nursing and post-acute care centers in the Knoxville market. This role is responsible for building strong hospital partnerships, supporting admissions growth, and coordinating smooth transitions of care from hospital to skilled nursing.

This is an excellent opportunity for someone with experience in healthcare admissions, case management, business development, or healthcare marketing who enjoys relationship-building and making an impact on both patient outcomes and facility growth.

What You'll Do

• Build and maintain strong relationships with hospital case managers, discharge planners, physicians, and referral partners
• Review referrals and coordinate admissions into skilled nursing facilities
• Drive census growth through strategic outreach and follow-up
• Coordinate smooth transitions from hospital to post-acute care settings
• Partner with facility leadership and admissions teams to support occupancy goals
• Represent Champion Care at hospital meetings, networking events, and community outreach opportunities
• Identify new referral sources and market growth opportunities

Qualifications

• Experience in skilled nursing, admissions, healthcare marketing, business development, or case management preferred
• Strong communication and relationship-building skills
• Ability to work independently and manage multiple priorities
• Knowledge of post-acute care or healthcare operations preferred
• Healthcare clinical background is a plus, but not required
