Care Manager II — Community Health Homes Lead

Hillside Family of Agencies is seeking a Care Manager II to support youth, adults, and families in community settings. You will lead assessments, care planning, and service linkage to reduce ER visits and Medicaid costs. This is a full-time role based in Warsaw, NY, with mileage reimbursement and regional travel across counties.
