Be part of something bigger—together, we can change lives and create lasting hope!Position: Care CoordinatorJob Type: PRN / Non-ExemptShift Details: 7:00am - 7:30pm / 7:00pm - 7:30am (required to work two 12-hour shifts per month)Work Setting: Crisis - Recovery Response Center; Peoria, AZSalary/Hourly Wage: $24.00 - $26.00/hourSpecialties / Licenses or Certifications:Bachelor's Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; orBachelor's Degree in any field and a minimum of two (2) years of behavioral health experience; orHigh School Diploma or equivalent and a minimum of four (4) years of behavioral health experience.Summary of Job:The Care Coordinator facilitates the quality and improvement of a person's experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.Essential Duties and Responsibilities:Guest Care & Advocacy: Provides coordination of care of services and resources available internally as well as throughout the community.Acts as an advocate for the needs and rights of individuals.Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community.Compliance & Risk Management: Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members.Keeps abreast of all federal and state regulations and laws and all Recovery Innovations policies and procedures as they presently exist and as they change or are modified.Understands and adheres to Recovery Innovations compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.Cross Functional Collaboration: Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner.Partners with other provider agencies in the coordination of care to help participants achieve wellness.Additional Duties: Performs other related duties as assigned.Your well-being and success are our priority at RI—explore our benefits and discover how we invest in YOU every step of the way!
