Hospice Case Manager — Inver Grove/Hastings Team

Allina Health is seeking a registered nurse for in-home health care services in Minnesota. You will provide comprehensive nursing assessments, develop and coordinate care plans with patients and families, and work with an experienced team to ensure high-quality outcomes. The role offers a 1.0 FTE schedule with 8-hour day shifts, every fourth weekend, and potential travel across the metro.
