Job Title: Case Manager Group: Programs Department: Supportive Services for Veteran Families (SSVF) Region: NC Location: NC regional offices (Durham, Fayetteville, Rocky Mount, or Salisbury) Classification: Non-Exempt Status: Full Time Reports to: Team Lead Travel Requirement: Telework capacity and travel in MD, VA, DC, and the Carolinas as needed. Job Summary Provide day‑to‑day support and services for participating veteran households. Requires experience in homelessness prevention, rapid re‑housing, and transitional supportive services. Serve as key liaison from intake through attainment or maintenance of permanent housing, promoting outreach, case management, advocacy, and wrap‑around supports with the VA and community partners. Responsibilities Coordinate outreach efforts for the SSVF Program, including establishing an outreach plan, developing schedules for visits to VA, shelters, and other venues frequented by homeless and low‑income veteran families, and maintaining communication with VA and key housing/community providers. Screen potential participants for eligibility using VA‑approved forms and verify veteran and income status with applicable agencies. Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan. Coordinate service delivery between the program and community partners for persons served and their families/supporters. Collaborate with persons served, families, housing agencies, and professionals to revise service plans and develop placement, stabilization, and follow‑up plans. Arrange housing support and wrap‑around services, connecting clients to VA and mainstream resources for transition to or maintenance of permanent housing. Travel to housing sites to support persons served, monitor progress, and address health, behavioral, or life‑skills issues. Communicate confidentially with clients and service teams to resolve service or support issues, especially regarding living skills, employment, education, family, and socialization. Ensure proper use and documentation of temporary financial assistance. Ensure veterans and families receive rapid re‑housing support, addressing assessed needs, promoting decision‑making, securing financial independence, and fostering community relationships. Conduct participant re‑assessments according to SSVF requirements. Participate in program and professional supervision, attend required staff meetings, Project Management Team meetings, and trainings, and contribute to team‑based planning. Complete reporting in HMIS and other systems as required. Complete other duties as needed and requested. Effect on End Result Rapidly addresses veteran housing, employment, family, social support, and life‑skill issues, adjusting services to maintain housing, economic stability, or benefits and community integration. Provides appropriate transitions and service plans and supports. Ensures complete and timely client file documentation and statistical reporting. Qualifications A bachelor’s degree in counseling psychology, rehabilitative counseling, or a related field. Service Field Is Preferred (or Comparable Experience) Experience working with homeless individuals and families, including veterans, is a plus. Other This position requires driving a personal vehicle for company business. Must meet Agency MVR guidelines. Valid driver’s license in jurisdiction of residence. Vehicle liability coverage if driving personal vehicle; may be required to transport clients. May be required to drive a van. Acceptable criminal background check and OIG exclusion check. Negative drug screening. CPR/First Aid Certification must be acquired and maintained upon employment. Physical Requirements Ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies. Physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic/home maintenance activities, and operate office equipment. Operating office equipment requiring continuous or repetitive hand/arm movements. Ability to remain seated for extended periods.
