Independent Home Health Nurse Case Manager

MJHS

MJHS Home Care is seeking a dedicated Registered Nurse to lead the development and management of comprehensive home health plans for patients recovering at home. You will coordinate care, assess needs, educate patients and families, and communicate with physicians and caregivers to ensure optimal outcomes. The ideal candidate holds an active RN license, 1+ year of acute medical-surgical experience, and a willingness to travel within the service area. #J-18808-Ljbffr MJHS
