Crisis Case Manager: Discharge & Aftercare Lead

Highland Rivers Behavioral Health is seeking a Case Manager to support adults in the Crisis Stabilization Unit. You will assist in discharge planning, link individuals to aftercare services, and engage families and community resources to improve outcomes.
