About the LPN Licensed Practical Nurse Role:

As a Licensed Practical Nurse (LPN), you will play a vital role in our front-line care team, providing compassionate and individualized care to our residents. You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans, ensuring our residents receive the best possible outcomes.

LPN Licensed Practical Nurse Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and providing wound care
• Develop and implement individualized care plans, in collaboration with our interdisciplinary team
• Monitor and report changes in patient status to the healthcare team
• Participate in quality improvement initiatives and maintain accurate records
• Collaborate with our therapy team to develop and implement rehabilitation plans
• Provide emotional support and counseling to residents and their families

LPN Licensed Practical Nurse Requirements:

• Licensed Practical Nurse (LPN) certification in the state of Pennsylvania
• Prior experience as a LPN preferred (new graduates welcome to apply!)
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced, dynamic environment
