Job Summary/Purpose:
The Health Enrollment Specialist supports the mission ofServiciosde La Raza by helping individuals, youth, and families gain access to affordable health care coverage. This role exercises independent judgment in assessing clients healthcare benefit needs,providingenrollment guidance, and coordinating referrals to ensureequitableaccess to Medicaid/CHP+, marketplace subsidies, and other low-cost programs. The Health Enrollment Specialist also develops strategies for outreach and education, tracks enrollment outcomes, andmaintainsaccuratedocumentation. By bridging gaps in coverage and advocating for underserved communities, this position strengthens client health outcomes and contributes to the agencys broader mission of equity and community well-being.
This position supports a community outreach initiative that promotes equitable access for people with disabilities by increasing awareness of handicap parking placards, reducing language and cultural barriers, and educating the Latino community. The Enrollment Specialist assists community members with completing and submitting applications for handicap parking placards, provides information about eligibility and the application process, and helps ensure that individuals with mobility disabilities can access the resources and services they need to improve their independence and quality of life.
JobDuties andKeyResponsibilities:
- Assess the healthcare benefit program needs of individuals, youth, and families, applying professional judgment todetermineeligibility andappropriate programs
- Facilitate enrollment in public health careassistance programs, including Medicaid/CHP+, marketplace subsidies, and other low-cost options
- Provide in-person, virtual, and phone assistanceregardinghealth insurance options, exercising discretion to tailor guidance to individual client circumstances
- Plan andfacilitatecommunity educationregardinghealth benefit options and the Affordable Care Act (ACA), adapting materials and approach to community needs
- Facilitate linkage to care throughappropriate internaland external referrals, including certified brokers and other community resources
- Support clients in the referral process for appeals, grievances, or complaintsregardinghealthcare toappropriate externalagencies
- Develop follow-up plans to ensure complete enrollment, retention, and identification of gaps in healthcare coverage
- Maintain knowledge of state coverage changes, policies, procedures, and resource availability, applying this knowledge to guide client decisions
- Accurately track and record all health enrollment applications and outcomes, ensuring compliance with agency standards.
- Provide input and recommendations on programmatic improvements based on client and community data
- All members of the SDLR workforce are expected to support the agencys community outreach, civic engagement, and recruitment efforts
- Support the agencys fundraisingobjectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agencyambassadorship, informational outreach, and public speaking
- Commitment to andexperience withequity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities
- Ability toestablishand maintain effective working relationships with clients, co-workers, agency partners, and the public

Required Education,experience,and Credentialing:
- High School Diploma or equivalent
- Two years of experience in a community-servingnon-profit setting preferred
- Bilingual English/Spanish
