You'll provide longitudinal, comprehensive care to a complex senior panel in skilled nursing and long-term care settings relationships that matter, not revolving-door medicine. $211K $317K base salary Longitudinal patient relationships Nevada no state income tax What you'll do Deliver comprehensive primary care to CareMore ISNP members with complex chronic conditions in SNF, LTC, and institutional settings. Conduct admission evaluations, routine follow-ups, and acute visits. Develop individualized care plans including chronic disease management and advance care planning. Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation. Collaborate with nurses, care managers, social workers, specialists, and facility staff. Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.
