Responsibilities
- Assess home health and hospice patients, in stable and emergent situations, and document observations to identify physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain and integrate data on physical, psychological, social, spiritual, and cultural factors into the POC.
- Maintain up-to-date patient records; ensure visits, contacts, problems, plans, actions, and goals are accurately recorded and updated.
- Provide care with appropriate infection control measures; educate patients and families on care, medications, safety, and self-care practices.
- Supervise and provide clinical direction to Home Health Aide (HHA), STNA, and LVN staff as needed to maintain care continuity.
- Assess and reassess pain; implement and educate on pain management strategies.
- Arrange referrals to appropriate agencies when indicated; monitor third-party payer requirements.
- Participate in interdisciplinary team meetings and communicate effectively with patients, families, and healthcare professionals.
- Perform on-call responsibilities as assigned and respond to urgent needs in the home setting.

Requirements
- Current state license as a Registered Nurse - Required
- CPR certification - Required
- OASIS experience - Required
- Homecare Homebase software experience - Required
- Current driver's license - Required; reliable transportation and insurance
- Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program; BSN - Preferred
- Experience: 1-2 years as an RN in a clinical care setting - Required; 2+ years of home health experience - Required
- CHPN certification - Preferred
