Position Overview

Bridge View Nursing Home is seeking an experienced and detail-driven MDS Director (RN) to lead our clinical reimbursement and assessment processes. This key leadership role ensures accuracy, compliance, and optimal resident care planning in a fast-paced long-term care environment.

Key Responsibilities:

• Oversee and manage the Minimum Data Set (MDS) process in accordance with CMS and NYSDOH regulations

• Ensure timely and accurate completion of MDS assessments, care area assessments (CAAs), and care plans

• Lead and mentor the MDS team and coordinate interdisciplinary input

• Maximize case mix index (CMI) and reimbursement through accurate clinical documentation

• Conduct audits of MDS and supporting documentation to ensure compliance and quality

• Collaborate with nursing, therapy, and administration to support resident-centered care planning

• Stay current with regulatory changes and implement best practices

Qualifications:

• Current RN license in New York State (required)

• Minimum 2–3 years of MDS experience in a skilled nursing or long-term care setting

• Prior supervisory or leadership experience preferred

• Strong knowledge of CMS guidelines, RAI Manual, and reimbursement systems (PDPM)

• Excellent organizational, analytical, and communication skills

• Detail-oriented with the ability to meet strict deadlines
