Key Responsibilities\n \n \n Conduct comprehensive clinical assessments for incoming patients to identify mental health needs and challenges. \n \n Create personalized treatment plans based on each patient's unique circumstances and therapeutic goals. \n \n Implement solution-based therapeutic interventions that draw on evidence-based practices. \n \n Maintain accurate and timely clinical records using our state-of-the-art electronic health record (EHR) system to ensure continuity of care and compliance with regulations. \n \n Collaborate with a multidisciplinary treatment team, including psychiatrists, nurses, and other social workers, to coordinate care effectively. \n \n Provide education and support to patients and their families, helping them understand treatment options and coping strategies. \n \n Stay informed of the latest research and developments in mental health to enhance your practice and improve patient outcomes. \n Patient Population / Client Focus\n In this role, you will work with a diverse range of patients, including children, adolescents, adults, and families facing various mental health challenges. Your expertise will be vital in providing support to individuals experiencing anxiety, depression, trauma, and grief—especially within the hospice care context. Understanding the unique needs of each client will enable you to implement effective care strategies that promote healing and resilience.
