Job Summary:
Hospice Registered Nurse (RN) - Colchester, Vermont. This assignment involves providing hospice care on a regional basis, with responsibilities including symptom assessment, medication management, and end-of-life care planning. The role supports a community hospice team, with scheduled shifts during the nights, and involves both on-call and direct patient care activities.

Job Details:

• Schedule: Nights, 12 hours per shift, minimum of 36 hours per week
• Required Qualifications: Certifications in American Heart Association Basic Life Support (AHA BLS); a minimum of 3 years of prior community hospice experience; proficient typing skills for narrative-heavy electronic medical records; experience in hospice/end-of-life care with complex symptom management and patient education; strong critical thinking skills in symptom assessment and management; comfort initiating and participating in care goals discussions with patients, family, and team; medication management and administration via IV, subcutaneous, oral, sublingual, rectal, and other routes; Epic electronic health record experience within the past 2 years

  • Schedule: Monday to Sunday, no block schedules for any reason
