Responsibilities
- Evaluate patients and develop individualized treatment plans to address mobility, strength, balance, gait, and activities of daily living.
- Implement skilled interventions, monitor patient progress, and adjust the plan of care to achieve goals.
- Document evaluations, treatments, and progress notes accurately; communicate findings to the care team and family/caregivers.
- Collaborate with interdisciplinary team members to coordinate care, discharge planning, and appropriate referrals.
- Educate patients and families on home exercise programs, safety, fall prevention, and activity modification to support independent functioning.

Requirements
- Active Physical Therapist license or eligibility to practice physical therapy in the United States.
- Ability to work in a home health setting and travel to patient homes.
- Strong communication, assessment, and documentation skills.
- Commitment to patient safety, professional standards, and compliance with licensure requirements.
- Willingness to travel within the service area and maintain reliable scheduling for home visits.
