PUBLIC HEALTH NURSE - CASE MANAGEMENT FOR HIGH-RISK PREGNANCY

Health Dept CC4C CPCM 200 South Post Rd Shelby, NC 28152 Job Description Summary: Performs professional work as a public health nurse with the responsibility of coordinating and assuring case management services are provided to pregnant women in Cleveland County within the Case Management for High-Risk Pregnancy program. Essential Functions and Responsibilities Conduct outreach through referring potentially Medicaid-eligible pregnant women to prenatal care, assess for care management needs, and conduct Medicaid eligibility determination. Maintain awareness of population identification through risk screenings data, ensuring all information is entered into the Virtual Health program and analyzing data to develop strategies to identify additional risks and patient needs. Complete comprehensive pregnancy assessments on all recipients based on program guidelines and provide care management support for identified population regarding stratification guidelines. Provide care management services in accordance with the approach outlined in the Case Management for High‑Risk Pregnancy Standardized Plan and provide referral to available community services. Develop patient‑centered care plans and document key activities within Virtual Health. Maintain integration and collaboration with health care providers within the county. Support and sustain collaboration within the local network, regional consultants, pregnancy medical homes, nonpregnancy medical homes, and the local community care network regarding challenges, risks and program goals. The responsibilities and duties outlined above are intended to provide a general outline and summary of this role. It is not intended to be an exhaustive or comprehensive list of duties and responsibilities. Additional tasks may be assigned as needed, with or without notice, to meet the needs of the department and county. Education & Experience Associates Degree in Nursing and 1 year of Nursing Experience; Or a Bachelor's Degree in Nursing. Knowledge, Skills, and Abilities Knowledge of and skilled in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs. Knowledge of available resources and organizations, current social and economic problems related to public health and the ability to coordinate as needed. Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions. Ability to secure the cooperation of clients, to elicit needed information, and to maintain effective working relationships. Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions. Work Environment / Physical Requirements This work requires occasional exertion of up to 10 pounds of force. Work regularly requires standing, walking, speaking or hearing and using hands to finger, handle or feel and occasionally requires sitting. Vocal communication is required for expressing or exchanging ideas by means of the spoken word. Hearing is required to perceive information at normal spoken word levels. Work requires preparing and analyzing written or computer data. Work has no exposure to environmental conditions. Work is generally in a moderately noisy location like a medical clinic. License & Certifications Active license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. Valid driver's license and insurable driving record. Work Location This role must work from the location identified at the time of hire with work travel required as needed. Any change of work location, even on a short‑term or interim basis, must be reviewed by the Supervisor and HR for approval. By General Statute 166A‑19.3, Departments of Health and Human Services are agencies that are a part of the State Emergency Response Team for occurrences or imminent threat of widespread or severe damage, injury, or loss of life or property resulting from any natural or man‑made accidental, military, paramilitary, terrorism, weather‑related, public health, explosion‑related, riot‑related cause, or technological failure or accident, including, but not limited to, a cyber incident, an explosion, a transportation accident, a radiological accident, or a chemical or other hazardous material incident. In the event that the State Emergency Response Team is activated, this position will be required to participate if deemed necessary. This is a safety sensitive position and subject to random drug screening. Compensation

$60,920 (COMPENSATION WILL BE BASED ON EXPERIENCE)
