Housing Case Manager: Help Residents Achieve Stable Living

Job Description The Case Manager works as a team member with several social service professionals and paraprofessionals, ensuring the delivery of quality care in accordance with rules and regulations established, and adhering to the mission of St. Joseph Center. Job Summary The Case Manager’s key areas of responsibility include providing high quality onsite case management services to residents of specific housing projects, serving as the central point of contact for clients as they transition from homelessness to tenanted community living.
ESSENTIAL DUTIES & RESPONSIBILITIES
Key Areas of Responsibility Provide high quality onsite case management services to residents of specific housing projects. Serve as the central point of contact for clients as they navigate the transition from “person experiencing homelessness” to “tenanted living within the community”. Evaluate individuals/families for permanent housing and assist in establishing eligibility for subsidized housing, including collecting required documents and completing applications. Essential Duties Develop professional relationships, maintain a caseload, and conduct regular face-to-face sessions. Assist with forms and documentation for housing voucher approval and annual recertification. Conduct initial and ongoing home and case management visits. Monitor progress, provide referrals for health, mental health, education, and vocational training, and advocate on behalf of clients. Educate on housing retention, social isolation reduction, and relapse prevention. Support development of independent living skills. Develop individualized service plans addressing various needs and risk factors. Facilitate medication and treatment appointment support. Utilize a harm reduction, client‑focused, strengths‑based model. Enter case notes into HMIS within 48 hours and submit weekly review notes. Manage client file organization. Respond to urgent client and landlord requests. Provide new resident orientation. Develop close relationships with collaborating agencies. Participate in collaborative and community meetings as directed. Other Duties Other duties and responsibilities may be assigned as needed. Qualifications Knowledge, Skills & Abilities Bilingual fluency in English and Spanish preferred. Highly motivated, self‑starter. Effective communication with diverse groups. Excellent organizational skills and ability to work in fast‑paced environments. Strong knowledge of housing market, programs and resources. Knowledge of harm reduction, motivational interviewing, critical time intervention, and housing first. Proficiency in Microsoft Office Suite. Internet browser proficiency. Experience 1+ years of experience providing services to highly vulnerable populations. Education Bachelor’s degree from an accredited college or university in social work, human services or related field; experience may substitute. Direct Reports This position has no direct supervisory responsibilities. Computer Skills Proficiency in MS Office Suite. Internet browser proficiency. Certificates, Licenses and Registrations None required. Travel Requirements Access to reliable transportation. Work Environment The work environment features moderate noise. Reasonable accommodations may be made for disabilities. Physical Demands Frequent standing, walking, stooping, talking, reaching, and occasional lifting of up to 20 lbs.
