Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A clinical infusion pharmacist opportunity is available to support home infusion services in the Pasadena, TX area. This contract role begins immediately with an expected duration of 8 to 12 weeks and offers a Monday through Friday schedule from 8:30 a.m. to 5:00 p.m., with possible on-call rotation responsibilities.

As a key member of the multidisciplinary home infusion team, you will prepare and dispense complex intravenous (IV) and subcutaneous medications in a sterile environment, ensuring strict adherence to state and federal regulations. Your role will include monitoring pharmacokinetic dosing, such as vancomycin, managing Total Parenteral Nutrition (TPN), and providing thorough patient and caregiver education on medication administration and infusion pump use. Collaboration with physicians, nurses, and pharmacy operations will be essential to optimize therapy management and reduce hospital readmissions.

Core Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions.
• Monitor laboratory values and adjust medication dosing.
• Educate patients and caregivers on infusion techniques and catheter maintenance.
• Coordinate care with healthcare providers and internal teams.
• Ensure compliance with regulatory standards including HIPAA, DEA, OSHA, and accreditation bodies.
• Manage inventory, including refrigerated products, and oversee medication shipping logistics.

Desired Qualifications:

• 1-3 years of experience in infusion specialty pharmacy or hospital IV room setting.
• IVIg experience is a plus.
• Familiarity with specialty therapy payer requirements and reimbursement processes.
• Current pharmacist license in Texas, in good standing.
• Proficient with CareTend pharmacy system.
