We are an intimate and unique program that provides personalized care to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients.This position will include field visits in Queens and Nassau counties, in addition to in-patient visits.Position OverviewReporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others.
