Case Manager

$24.04 - $27 per hour

Community Reach Center

About the Role Case Manager works in the Outpatient, Community programs, and Case Management Teams, supporting individuals to develop social, occupational, leisure, and living skills. Responsibilities Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families Supports and closely coordinates mental health care with the consumer’s clinical care team and the primary care provider Attends collaborative treatment team meetings and reviews of consumers Provides crisis de-escalation when needed Assists eligible consumers in gaining access to needed medical, social, educational, and other services Provides an assessment of needs and service plan development Empowers consumers to reach their goals and serves as a role model and technical expert to help them achieve objectives Provides transportation for consumers in agency vehicles when needed Works closely with outside organizations on resources and care coordination Uses an evidence-based model of measurement-based care to track success for each individual consumer Qualifications Bachelor’s degree in Human Services or a related field Strong communication, organization, time-management, and clinical skills Ability to work independently in a community-based setting Valid Colorado driver’s license with a clean driving record Bilingual Spanish preferred, but not required Physical Requirements Regularly stand, sit, talk, hear, and use hands and fingers to operate a computer and keyboard Requires clear close vision for computer work Light to moderate lifting is required Regular, predictable attendance is required Schedule Monday-Friday 8 am–5 pm with a 1-hour unpaid lunch; work in office and community, travel and vehicle use required. Salary $24.04 to $27.00 per hour; eligible for $250/month bilingual stipend.
