Work Environment

As a Physical Therapist at Slate Care, you will work primarily in patients' homes, allowing you to establish a unique rapport and understanding of their individual needs. The home care setting provides a comfortable atmosphere for rehabilitation, enabling you to deliver therapy in a familiar environment where patients can easily integrate practices into their daily routines. Our flexible work arrangement allows you to choose your hours between Monday and Friday, promoting a healthy work-life balance.

Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical and functional capabilities, ensuring personalized treatment plans are developed effectively.
• Implement diverse treatment methods, including therapeutic exercises, stretching routines, manual therapy techniques, and specialized equipment, to enhance mobility and alleviate pain.
• Carefully track and document patient progress, responses to treatments, and adjust plans as necessary to meet evolving therapeutic goals.
• Educate patients, caregivers, and family members on effective physical therapy practices and assistive devices that promote autonomy and self-care.
• Regularly engage in professional development activities to stay current with best practices and advancements in physical therapy methodologies.
• Collaborate with multidisciplinary teams to ensure a cohesive approach to patient care, sharing insights that can help improve overall treatment outcomes.

Patient Population / Client Focus

Our patients range from children to adults recovering from injuries, surgeries, or chronic conditions. You will work with individuals who may require assistance in building strength, improving mobility, or managing pain. Each case is unique; therefore, your support will be tailored to meet the specific needs and goals of each patient, allowing them to achieve their optimal potential.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• State License to practice as a Physical Therapist.
• Strong assessment and communication skills, both written and verbal, to effectively interact with patients and their families.
• Proficiency in using technology, including word processing and relevant therapy software.
• Current CPR certification, demonstrating your commitment to patient safety and readiness.
• Ability to adapt to changing patient conditions and schedules, showcasing flexibility and agility in your approach.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) is preferred, although 2 years of experience in the field is a minimum requirement.
• Experience in home care settings is advantageous, providing insights into creating comfortable and effective rehabilitation environments.
• Completion of continuing education courses related to physical therapy practices can enhance job readiness and competency.
