Details Home Health setting covering the Anacortes, Oak Harbor, and Coupeville territory Day shift schedule, 8am-5pm, Monday-Friday, 5x8 hour shifts No weekend or on-call requirements Requires in-office visits at least once every one to two weeks for team meetings, supplies, and admission packets Caseload of approximately 5-7 patients per day with 6 visits daily and a minimum of 30 productivity points weekly Patient population primarily geriatric Territory extends up to a 60-mile radius from the main office and may require ferry travel Uses HCHB EMR system iPad provided for documentation; personal cell phone not required Mileage reimbursed at the IRS standard rate Requirements Minimum 1 year of recent Home Health experience required BLS certification required OASIS experience required HCHB EMR experience required License required if applicable Experience with orthopedic, neurologic, and post-operative patient populations preferred Skilled in home exercise program development and patient/family education Must be flexible with territory assignments Timely documentation and communication required Additional Information Responsibilities include home-based patient assessment, treatment planning, and rehabilitation for geriatric patients Scrubs required in any color; no jeans permitted Local candidates accepted at the travel rate First-time travelers will not be considered for this position American Traveler Job ID P-750073.
